Appointments

To schedule an appointment, call (616) 394-3788. School, camp and sports physicals and well-child and routine adult visits should be made well in advance. If you are unable to keep your appointment or will be late, please call our office to cancel or reschedule. Please make every effort to keep your appointment. Time is available each day to treat acute illnesses or urgent problems. 

We request that you call our office early in the day if you wish to be seen that same day. A parent or legal guardian must accompany any child being seen for a physical or immunizations. If your child is ill and someone else is bringing him / her into the office, please include a signed permission slip so we can examine and/or treat your child. 

We will do our best to maintain a schedule that does not result in long waiting periods. Please understand, however, that emergency patients and other factors may cause unplanned changes in our schedule. 

We ask for your patience during these times, and if you are unable to wait, please tell our receptionist. In such cases we will conveniently reschedule your appointment if necessary.

Patient Portal

The MyChart patient app is a free software application you can download to your smartphone or tablet. Use it to access and view your Holland Community Health Center medical information and related care such as view test results, request prescription refills, message your healthcare team between appointments and more.

Same-Day Appointments

Time is available each day to treat acute illnesses or urgent problems. Please call our office early in the day if
you wish to be seen that same day. If your child is ill and someone else is bringing them into the office, please
include a signed permission slip so we can examine and treat them.

After-Hours Care

For urgent situations occurring after business hours, please call our office. Your call will be connected to the on-call nurse who can help you determine appropriate home-care interventions or if you should seek further care in the emergency room. For all life-threatening medical emergencies, always proceed directly to the nearest emergency room.

Prescriptions

Please ask your health care provider to refill any routine medications during your visits to the Center. If you do need a medication refill between appointments, please phone us during regular office hours. Plan ahead. Medications will not be refilled on the evenings or weekends except in emergency situations. When requesting a medication refill, please provide your name, the medication name and the name and phone number of the pharmacy you prefer. Be aware that medication refills may take up to 48 hours to be completed.

Payment & Insurance

No one will be denied medical care based on inability to pay at the time of service. We accept the following methods of payment:

  • Medicaid and Medicare
    • Identification card must be shown at each office visit.
  • Priority Health, Blue Cross/Blue Shield, Blue Care Network
    • Identification card must be shown at each office visit.
  • Commercial Insurance (Aetna, Travelers, and others)
    • Payment is expected at the time of service. A copy of your bill will be provided to you for reimbursement from your insurance company for covered services.
  • Private Pay / Sliding Fee Scale
    • Payments for health care services will be determined based on your ability to pay.
    • Please bring two (2) of your most current proof of income (check stubs, etc.) to assist us in establishing your sliding fee scale.
    • If your insurance carrier requires a co-payment, please pay that amount during service.
    • We accept cash, checks, Visa, MasterCard, and Discover
